THE FOLLOWING INFORMATION IS REQUIRED TO BE INCLUDED WITH YOUR RECORDS
- The
date of original production of the depiction
- The
title or identifying number of the depiction
- A copy
of any URL associated with the depiction. If no URL is associated with
the depiction, the records must include another uniquely identifying reference
associated with the location of the depiction on the Internet;
- The
legal name and birthdate of each performer
- Any
name ever used by the performer, including a maiden name, alias, nickname,
stage name, or professional name;
- A
legible scan of the performer's identification document. The scan must
be no smaller than the original size of the document. The filename
should equal the performer's first name and last name, separated by a hyphen
(-). Allowable formats are .pdf and .jpg; and
- A copy
of the depiction. Pictures must be in .jpg or .jpeg format. Video
must be in a non-proprietary format. Allowable formats are .mpeg, .wmv,
.avi, and .mov. No other file types are supported. Multiple image
files must be zipped into one archive (.zip). The filename must equal
the title or unique identifying number of the depiction. Use the hyphen
in place of spaces. For live internet
performances, the records must include a copy of the depiction with
running-time sufficient to identify the performer to associate him or her with
the records needed to confirm his or her age.
